- A referral from an academic teacher or employer or an opinion of an attendant of a specialisation selected by the student;
- Language certificate(s) or a certificate from a language teacher
- A confirmation of the average grade obtained throughout the whole period of university education (available at the Student’s Afairs Office)
The documents should be submitted in the room no 614 (6th floor)
Note! If you have already found an institution ready to offer you traineeship, do not forget to attach the letter of intent or proposal of „Learning Agreement for Traineeship” from this institution to your application.

- Language certificate(s) or a certificate from a language teacher
- A confirmation of the average grade obtained throughout the whole period of university education (available at the Student’s Afairs Office)
- Learning Agreement (One of the elements of preparation for Erasmus mobility is the preparation of the Learning Agreement – Agreement on the study program. UEHS in Warsaw uses primarily the electronic version of this document, i.e. OLA (Online Learning Agreement). The student receives logging data from the University and prepares OLA on the platform: https://learning-agreement.eu/
